Case Manager
Overview
Trinity Center is a non-profit organization which operates as a day-center for individuals experiencing homelessness in downtown Austin. The “Case Manager” Position is for 40 hours, from 8am-4pm, Monday through Friday. The case management program at Trinity Center has a strong focus on housing, accessing resources, holistic well-being, financial stability, and moving toward goals related to ending the person’s experience of homelessness.
Responsibilities
Under the Direct Supervision of the agency’s Case Management Supervisor Essential Duties Include:
Providing support in implementing the case management program: Includes updating and maintaining case management policies and practices to best support client needs,
Providing direct case management for unhoused individuals, maintaining a caseload of 15-20 individuals regularly,
Inputting and updating all case management data for reports on a regular basis,
Providing and researching current and most effective community resources, and
Participating in advocacy within the community for clients.
Staffing a Sunday 2:00-4:30PM shift is required 4-5 times each year for the agency’s Sunday Street Ministry—Time is compensated the following week.
When not engaging in Case Management client activities, the Case Manager will assist with Center’s operations, specifically at the front desk, especially for resource referrals and “*Quick Case Management/Resource Navigation*” (See note below)
Following Training/Certification, conduct Coordinated Assessments for clientele at least once a week.
Following Training/Certification in SSI/SSDI Outreach, Access, and Recovery (SOAR), assist clients with applications.
Perform other duties as assigned
**Quick Case Management/Resource Navigation refers to duties/tasks such as resource referrals, assisting with filling out of applications, assisting with making phone calls alongside a client, assisting with items on the computer, etc.—tasks and duties that are Case Management-related, but would not require ongoing meetings or work past that instance.
Knowledge/Skills/Abilities:
· Bilingual proficiency (Spanish/English) preferred.
· Proven experience of working with people who are homeless or at imminent risk of becoming homeless
Experience providing case management, knowledge of community resources
High degree of knowledge and competency in mental health and substance use challenges
Knowledge of various clinical practices and frameworks such as cultural competence, the recovery model, housing first, and motivational interviewing
Assure that individuals are receiving needed services, evaluate the effectiveness of services, and determine if needs are being met
At ease working in a faith-based organization.
Willingness and ability to step out of the exclusive role of a case manager. All employees at Trinity Center assist in the general operation of the center.
Highly organized: Demonstrates exceptional ability to manage multiple case files, deadlines, and administrative tasks efficiently, ensuring accuracy and completeness in documentation and reporting.
Self-starter: Initiates proactive solutions and improvements in case management processes, demonstrates initiative in addressing client needs, and takes ownership of tasks from inception to completion.
Able to work independently: Capable of making informed decisions autonomously, prioritizing workload effectively, and maintaining a high standard of service delivery without constant supervision
Qualifications
MINIMUM EDUCATION:
· Bachelor’s or Master’s of Social Work
· Licensing is preferred, but not required
REQUIRED COURSES/COMPLETIONS (e.g., CPR):
· Trained and certified to utilize ECHO’s Homeless Management Information System (HMIS) and perform Coordinated Assessments for clients*
· Trained and certified in SSI/SSDI Outreach, Access, and Recovery (SOAR) to be able to assist clients with applications*
· Mental Health First Aid*
*Courses/Completions can be paid for and provided by employer if needed
SALARY AND BENEFITS:
Base Salary Range: $50,000-$56,000
The salary offered will be based on candidate’s education and experience.
